IMPORTANT NOTICE: STUDENT UNDERTAKING FORM

All students are hereby directed to download the Student Undertaking Form, print it, and ensure it is duly signed by their parent/guardian before resumption to school.

Submission of the completed undertaking form is compulsory.

Any student who fails to submit the form will NOT be welcomed or recognized as a student of the institution upon resumption.

Furthermore, be informed that copies of the submitted undertaking forms will be forwarded to the Police Station and other relevant security agencies for official record and security purposes.

All students are advised to comply strictly with this directive.
