1. Original O’Level Result(s)

– WAEC / NECO  (maximum of two sittings)

2. Birth Certificate / Declaration of Age

3. Local Government Identification Letter

4. Medical Fitness Certificate (from a government-recognized hospital)

5. Admission Letter

6. School Application / Screening Slip

7. Recent Passport Photographs

– At least 4–6 copies

8. Evidence of Payment

– Application fee / Acceptance fee / Screening fee (if applicable)

Course Change Evidence

(for candidates who changed course or institution on JAMB)

9. File

– Brown file or departmental file for documentation

10. Writing Materials

– 2 A4 Multi purpose paper, 2 pack of Marker, and 1 bottle of printer toner 

IMPORTANT NOTES

1. All candidates must come with both originals and photocopies of each document.

2. Candidates who fail to present required documents will not be screened.

3. Screening is compulsory for all newly admitted students.
